This Ash is received as a reward for turning in your fourth Deathroot to the Beast Clergyman in Caelid — see Deathroot. It can also be found north of the Ravine-Veiled Village site of grace in Liurnia of the Lakes — see Ravine-Veiled Village walkthrough.
